Parent/guardian consent

• I confirm that my child is physically fit and healthy, and I will advise you of any changes. I

consider him / her capable of taking part in gymnastics. I have completed the section on

medical details and give consent that in the event of any illness / accidents that require

first aid treatment can be administered.

• By completing this agreement I declare that I am aware of the element of risk involved

and whilst I accept that the coaches and event personnel will take precautions to prevent

accidents.

Booking policy

• I am aware that all classes are payable, even if the member does not attend a class

through illness or holidays, and that in rare circumstances when we have to cancel a

class/es due to severe weather conditions there will be no replacement classes or

refunds offered.

• I understand that following a free trial session, I will have priority booking for 5 days and then my child will be removed from the list.

Photography and videography

• I understand that if I wish to film, video or photograph my child at Spring Boxx I must

first obtain permission from the lead coach and ensure I only film/video/photograph my child.

• I will not publish any images/videos taken at Spring Boxx in any way and more

particularly on the internet/social media for any purpose unless they contain only my child.

• I will not use the photographs/videos for any reason that could be derogatory to the

gymnasts, coaches, other gymnasts’ abilities or to discredit the club.

I acknowledge that Spring Boxx’s policies can be found on the website (i.e. safeguarding, code of conduct, accidents and incidents, bookings, equity and bullying, privacy, photography). By completing this agreement I confirm membership to Spring Boxx for myself and the named dependent.
